Per correspondence received in this office on July 6, 2023, at 8:55 a.m., there has been a United States Postal Office city change for this facility. The city is being changed to Hazle Township; effective July 6, 2023. The official complete address for this facility is: 45 Quality Road, Hazle Township, PA 18202. (This address was verified directly with the United States Postal Service on Saturday, July 1, 2023.) (Upon inquiring about the official city, the facility recently informed our Division of having knowledge of this change as early as July 15, 2008.) An updated CMS-1513 was obtained.
Step by Step Inc. Quality remained in compliance with the Requirments of 42 CFR, Part 483, Subpart I Regulations for Intermediate Care Facilities for Individuals with Intellectual Disabilities.
